Just a thought, if I missed the obvious apologies in advance, Can't you just download the application form!!
It's not available from the Kathmandu website but it's the same form, so why not download it from another website??

It does not matter if you can download the form.
You have to go to the embassy two times anyway, the first time to hand in the telex form (they use it to enquire in your home country about you),
and about 3 days later the second time to hand in the actual visa application form.
They give you the application form on the first visit, so you have 3 days time to fill it in. No need to get the form in advance.

One thing I cannot let pass is OP mensioned in one of the posts that "I do not like to pay bribe", this was in reference to use of agent. Agent fees are not bribes, they are service fees!
