Your doctor will advise you which which vaccinations you need, if any.

He or she wills give you a Vaccination Card for your own personal record. It reminds you what you have had, and when you need boosters.

The Indian authorities do not ask to see your medical record when you enter the country.

However,  you may want to take a list of the vaccinations you have had in case you need medical treatment whilst overesas.

At this time (from the US) there are not required shots.  It just depends on where you are going and what you are going to be doing.
